Clinical Skills Demonstrate ability to engage proactively with patients and their families/ carers/ significant others to build positive working alliances. Ability to work with assessed needs embracing the wishes of the patient within a Tier 4 specialist CAMHS inpatient setting. Demonstrates the skills and knowledge essential for developing nursing risk management plans, and timely review of the risk management plans, identifying opportunities for positive risk taking in conjunction with the patient, Wider MDT and patients carers/relatives/significant others. Has up to date knowledge and skills in relation to role and responsibilities of the practitioner under the Mental Health Act 1983 and its application in practice and to be able to support junior qualified staff in the same way. Actively engages with the Case manager under CPA to identify appropriate resources to support the patients care package within the hospital setting and within their local environment. Works to develop effective multidisciplinary relationships through skilled communication with the unit and the wider CAMHS network including NHSE as required. Shows ability to effectively evaluate plans of care in collaboration with the patient as an ongoing process and make recommendations for changes in the care package. Work as a Senior Nurse in a Unit area ensuring a safe and therapeutic environment for patients, their families and visitors Applies relevant policies and statutory requirements within the unit and hospital framework including governance and communicates these requirements to the wider team. Respects and applies the requirements of equality and diversity, promoting and role modelling these across the team. Acts as a role model to promote a culture of learning development vision. Implements, monitors and evaluates therapeutic interventions within an overall care programme, enabling patients to develop coping strategies within the context of Positive Behavioural Support. Is named nurse for a small group of patients, acting as Associate nurse for colleagues when on duty, and is a regular shift leader. Who will be able to offer support and guidance to band 5 staff in the delivery and development of care. Work alongside the Case Manage, acting as an associate in the coordination of care with internal and external multi agency network including NHSE and the CCG where appropriate. Provides Clinical and managerial Supervision to a group of defined nursing staff Undertakes Lead roles for the unit, as agreed with the Matron Deputises for the Matron/Service lead as required Adheres to all National and local standards, policies and procedures. Demonstrates expertise in analysing and reflecting upon their own clinical practice and that of others in all aspects of care giving to young people and their families/carers. Supports the Matron/Service lead in developing sustainable services in line with national specifications, local implementation requirements and a set financial envelope. Promote and introduce systems to continuously improve quality of care, drawing on national and local guidance, and able to lead on key elements in particular CQC and QNIC requirements. Engages in the assessment of Children as part of the Units responsibility to NHSE gatekeeping responsibilities at times leading on key areas/cases in line with NHSE referral procedures and response times. Leadership Skills Actively works with the Matron and service lead to implement the vision for Tier 4 inpatient care. Including new ways of working and associated outreach and outreach plus were required. Actively engages in the clinical audit process and local audit projects. Actively engages willingness to participate in the service evaluation. Acts as a role model in the clinical arena for other staff. Demonstrates the knowledge and skills necessary to provide effective clinical and managerial supervision of the shift co-ordinators and junior staff on the team. Actively involved in Service Improvement initiatives within the area of work and as required by the Community and Mental Health Division. Management Skills Demonstrates the commitment to develop the range of skills required to deputise for the Matron Demonstrates the skills of effective time management and ability to prioritise competing demands. Able to work positively with patients and/or carers who express verbal concerns or complaints about the service and attempt early and effective resolution in conjunction with the PALS Service. (Would seek support from Matron and service lead). Plus follows Trust policy and procedures in the management of complaints and reporting of same. Is cognisant of the policy and procedural guidance following a Serious Untoward Incident on the unit and able to prepare a clear and concise report as required. Is cognisant of local Trust policies and adheres to policy and procedural guidance and enables development of team in their understanding and application of policy and procedural guidance. Shows awareness of the major national policy initiatives and their influence on care delivery. Actively assists and at times take a lead approach to assist the Matron/Service lead in the management of change, taking account of the views of patients, carers and the unit MDT. Keeps up to date with legislation, evidence-based practice and has a clear understanding of good practice. Identifies deficits in skill mix and safe staffing levels and acts to address these, will have responsibility for producing a unit roster and monthly safe staffing returns and internal payroll procedures. Demonstrates the interest and enthusiasm essential for developing mentoring relationships with students on placement on the unit, registered nurses in preceptorship and junior staff including Health care assistants, and Assistant Practitioners. Shows evidence of commitment to lifelong learning through ongoing professional and career development. Follows professional guidelines and Codes of Practice as laid down by the NMC and ensures that all registered staff follow the same. To actively participate in an annual performance review (appraisal) and the development and implementation of a personal development plan. Clinical and Managerial Supervision of a defined group of team members and contributes to their overall CPD/PDR, mandatory training and appraisal process recognising the skills required across the team. This will also include addressing any performance and attendance issues that are below the expected standard. To take responsibility for own professional development ensuring professional standards are maintained and statutory and mandatory training is in date. To attend any training as requested. To actively participate in training and learning and development opportunities in line with the role/remit of the post. Will be required to be on call for the unit at the direction of the Matron and or Service lead Will follow an out of hours on call rotation for weekends/bank holidays with other band 6 Team Coordinators to provide senior cover to the service where required This job description is not restrictive or definitive in any way and should be regarded only as a guideline to the duties required and may be amended in the light of changing circumstances following consultation with the post holder.
